One of the first steps towards Baptism and/or Church Membership at Parkway is to complete the appropriate application form and submit it to the church office.   Obtain the application below, print it, and submit to the Church Office.

MINISTRY APPLICATION

Everyone who works in the Parkway Ministry must fill in a Ministry Application.  and if working with children or youth a Police Records Check Form.    You can print an one by clicking on Ministry Application or you can obtain a copy from the church office.   You must obtain the Police Records Check application from the Church Office.  Once you have completed both the Ministry Application Form and the Police Records Check,  please return both to Pastor Rick's church mail box.
